Dental Crowns and Dental Implants

A Brief History

Dental crowns and dental implants both have ancient origins, with early versions seen in ancient Egypt and among the Etruscans, who used gold to restore damaged teeth. Modern dental crowns, developed in the late 19th and early 20th centuries, are made from materials like ceramic, porcelain, metal, and resin, offering both aesthetic and functional benefits. Dental implants also have ancient roots, with early versions using materials like shells and metals. The modern era of dental implants began in 1952 when Swedish orthopedic surgeon Per-Ingvar Brånemark discovered that titanium could bond with bone. This led to the development of titanium implants, now widely used to replace missing teeth, providing a reliable foundation for fixed or removable replacement teeth.

So What are They?

Both Crowns and Implants are types of restorative dentistry. We’ve explained an overview of restorative dentistry in our last post, so we’ll spare you the details, and just keep it relevant to this post. It is common for people to mix up crowns and implants and it is important to know the distinction before you decide which is right for you.

Dental Crowns

Dental crowns, often referred to as “caps,” are prosthetic devices placed over damaged or decayed teeth to restore their shape, size, strength, and appearance. Here are some key facts about dental crowns:

  1. Materials and Construction:
    • Crowns are typically made from ceramic, porcelain, metal, or a combination of these materials.
    • They are custom-made to match the color and contour of your natural teeth, ensuring a seamless appearance.
  2. Application and Purpose:
    • Crowns are used to protect weakened teeth, restore broken or severely worn-down teeth, and cover teeth with large fillings.
    • They are also used to cap teeth after root canal treatment, to hold a dental bridge in place, or to cover misshapen or severely discolored teeth.
  3. Procedure:
    • The process involves reshaping the existing tooth, taking an impression for the custom crown, and then cementing the crown over the prepared tooth.
    • This procedure typically requires two dental visits: one for preparation and another for placement.
  4. Benefits:
    • Dental crowns improve the strength, function, and aesthetics of damaged teeth.
    • They provide long-term durability, typically lasting between 5 to 15 years with proper care.

Dental Implants

Dental implants are artificial tooth roots made from biocompatible materials like titanium, which provide a strong foundation for fixed or removable replacement teeth. Here are some key facts about dental implants:

  1. Materials and Design:
    • Implants are usually made from titanium due to its ability to integrate with bone through a process called osseointegration.
    • The implant itself is a small post that is surgically placed into the jawbone.
  2. Application and Purpose:
    • Dental implants are used to replace one or more missing teeth, providing a stable base for artificial teeth such as crowns, bridges, or dentures.
    • They help preserve bone structure and prevent the shifting of adjacent teeth.
  3. Procedure:
    • The implant procedure involves placing the implant post into the jawbone, allowing time for osseointegration (typically a few months), and then attaching an abutment and crown.
    • This multi-step process ensures a secure and natural-feeling tooth replacement.
  4. Benefits:
    • Dental implants offer a permanent solution for missing teeth, with a success rate of up to 98%.
    • They look, feel, and function like natural teeth, providing stability and confidence in eating, speaking, and smiling.
    • Implants do not impact adjacent teeth and reduce the risk of further dental issues, such as bone loss.

Main Considerations

Dental Crowns

Crowns are ideal for restoring damaged teeth, offering a cost-effective solution with stable and aesthetic results. They are less invasive and quicker to implement, making them suitable for individuals with less severe dental issues.

Pros:

  1. Stability and Functionality:
    • Dental crowns provide stable chewing and speaking functions, similar to natural teeth.
    • They prevent adjacent teeth from shifting out of position.
  2. Aesthetic and Practical Benefits:
    • Crowns are designed to match the appearance of real teeth, enhancing your smile.
    • They can help align teeth and preserve the bite.
    • Crowns are easier to clean compared to removable dentures, as they can be brushed and flossed like natural teeth.
  3. Cost-Effective:
    • Dental crowns are generally less expensive than dental implants.
    • They are ideal for restoring damaged teeth, especially after procedures like root canals.

Cons:

  1. Longevity and Durability:
    • Dental crowns need to be replaced after a few years due to wear and tear.
    • The bone tissue under the bridge and crown can shift, reducing the overall durability of the restoration.
    • Surrounding teeth may weaken over time, becoming prone to decay.
  2. Gum and Tooth Issues:
    • There is a risk of gum recession at the margin of the crown, which can lead to sensitivity and aesthetic concerns.
    • There is a potential for a mismatch in color if natural teeth change shade over time.
    • The porcelain material of the crown can fracture, requiring repair or replacement.

Dental Implants:

Implants, on the other hand, are a more permanent solution for missing teeth, providing long-term benefits in functionality and appearance. They are particularly advantageous for those seeking a durable and natural-looking tooth replacement, despite the higher cost and longer treatment time.

Pros:

  1. Longevity and Natural Appearance:
    • Dental implants typically last longer, up to 25 years or more, compared to crowns.
    • They offer a natural look and feel, closely resembling real teeth.
  2. Functionality and Stability:
    • Implants function like natural teeth without affecting adjacent teeth, as they are standalone solutions.
    • They provide stability without the need for grinding down nearby teeth.
    • Dental implants help maintain facial bone structure and natural contour, preventing bone loss.
  3. Maintenance:
    • Implants are easier to clean and maintain, just like natural teeth, as no special cleaning solutions are required.
    • They do not slip or shift, ensuring ease in chewing and speaking.

Cons:

  1. Surgical and Healing Process:
    • The surgical procedure for dental implants is more involved and takes longer compared to crowns.
    • The healing time for implants is extended, often requiring several months for complete integration.
  2. Cost and Insurance:
    • Dental implants are more expensive than crowns and may not be fully covered by insurance plans.
    • The overall implant process can take up to nine months, requiring multiple dental visits and follow-up appointments.
  3. Potential Complications:
    • There is a risk of complications within a 16-year period after implant placement, including infection or implant failure.

How do you know what is right for you?

Deciding between dental crowns and implants can be challenging, as both options have unique benefits. The first step is to consult with a dentist who can evaluate your oral and physical health, including your teeth, gums, and jawbone structure. Factors like chronic diseases or habits such as smoking can influence the healing process and the suitability of dental procedures. Crowns are typically recommended for damaged but salvageable teeth, offering cosmetic improvements and protection, while implants are used to replace missing teeth entirely, providing a permanent solution. Additionally, consider your long-term dental goals, maintenance preferences, and budget, as crowns are generally less expensive than implants but may require replacement after several years.
